We investigate reported errors, prioritise issues that could affect health or care access, document material corrections, and provide a route for organisations and professionals to submit evidence.

How to report

Use the contact route to report a factual error or dispute a directory record. Include the page address and a public source where possible. Do not include symptoms, medical records, photographs, or other private health information.

What happens next

  1. We acknowledge receipt.
  2. We identify whether the issue is medical, factual/directory, legal, or technical.
  3. For a plausible high-risk claim, we may temporarily suppress the disputed detail while we review.
  4. We verify evidence, record the decision, and update the page version history.
  5. When a change materially affects understanding, we show a visible correction note.

Organisations and professionals can submit evidence to correct or dispute a profile. We verify changes against authoritative sources before publishing them.
