We investigate reported errors, prioritise issues that could affect health or care access, document material corrections, and provide a route for organisations and professionals to submit evidence.
How to report
Use the contact route to report a factual error or dispute a directory record. Include the page address and a public source where possible. Do not include symptoms, medical records, photographs, or other private health information.
What happens next
- We acknowledge receipt.
- We identify whether the issue is medical, factual/directory, legal, or technical.
- For a plausible high-risk claim, we may temporarily suppress the disputed detail while we review.
- We verify evidence, record the decision, and update the page version history.
- When a change materially affects understanding, we show a visible correction note.
